Channel of DOORDARSHAN Network, where in Dr.Mahesh Desai (Chairman Dept.
of Urology, MPUH), Dr.Mohan Rajapurkar (Head, Dept. of Nephrology, and
MPUH) and Dr.Sishir Gang (Consultant Nephrology, MPUH) were invited to
provide information on Kidney Failure and Renal Transplantation. The 30min.program
'Hello Doctor' was telecast on June'6'03.

Laparoscopic Donor Nephrectomy - The MPUH experience |
| |
Laparoscopic live donor [LDN] nephrectomy
has the advantages of reduced pain and shorter recuperation for the donor
without compromising the out come in the recipient. Gill et.al. demonstrated
the feasibility of performing LDN in porcine model and thereafter Ratner
et.al. reported the first successful human laparoscopic LDN . We started
doing LDN at out institute in November 2002.
50 donors were subjected to left nephrectomy and standard
transperitoneal laproscopic technique was followed with 3 ports in 7 and
4 in 43 patients.
Donor parameters evaluated were operative time; warm ischemia [WIT] and
total ischemia time [TIT]; HCT drop; analgesia required; start of orals;
days of hospitalization; post op complications.
Mean age was 44yrs Average WIT was 7 min. [4-11min.]
and TIT was 52 min. [39-79 min.]; Average laparoscopy time was 176 min.
[90-270min.]; average drop in haematocrit was 3.7% [1-8%] with only one
patient requiring transfusion. None of the donors needed conversion to
open surgery. Intra operative complications seen were lumbar vessel bleed
in 2 and splenic capsular tear in 1. All were managed laparoscopically.
Mild surgical emphysema was seen in 2 patients in postoperative period.
Average hospital stay was 3 days [2-6 days].
Laproscopic live donor nephrectomy is safe, effective
procedure with minimal morbidity for the donor. All the recipients had
immediate urine output and good early graft function. |

The 98th AUA was held in McCormic Convention Center. More
than 16000 delegates attended it. There were 1879 presentations, which included
121 videos, 11 special scientific sessions and 69 postgraduate/instructional
courses.
There were more than 18 urologists from India and about 80 Indian urologists
from settled in USA.
For the 1st time a coveted 'SIU Lecture' was delivered
by an Indian urologist, Dr.Mahesh Desai. Dr.Carl Olsson and Dr.Mebust
introduced him as "the most outstanding endourologist in the world."
AUA Urolithiasis Panel recommends health and economic outcome of treatment.
Dr.Mahesh Desai, in his opening remark said, "Stone Free status is
global but Economic consideration are very vital." He delivered a
spell bounding lecture on stag horn calculus - "Sandwich or Multiple
bite-an Asian experience" in 25 minutes. He showed very vividly the
importance of complete clearance by one or multiple tracts and stressed
that it may need several sittings to clear it totally. This was beautifully
illustrated in his last case of a giant stag horn in solitary kidney.
It took 11 sessions to render the patient stone free. I was sitting behind
Dr.Fuch and Dr.Lingman who complemented him by saying "this will
change the entire perspective of treating stag horn calculus world wide."
That was indeed a moment for Indian urology…………………………………
